The agreement form covers all of the policies and terms relating to the Personal Training service. It protects the Personal Trainer and client and eases communication by laying everything out in writing at the outset of the training relationship.
This agreement ensures that the role of the trainer to client and client to trainer is clearly appreciated and understood. This agreement must be signed prior to beginning the training sessions.
PAYMENTS · All payments must be made prior to any sessions being undertaken. Failure to make payment will result from in cancellation of payment the personal training package. · Payments can be made by bank transfer, standing order or cash.
Session Cancellations
We require 24-hour notice of any cancellation of sessions. This can be made directly to trainer, by email, txt message or phone call. If 24-hour notice isn't given, you will lose that session, and no refund will take place. (this is in place as we are extremely busy and to ensure other clients can have available slots. It becomes extremely hard to plan our week if we have clients cancel last minute).
Lateness
If trainer is more than 15 minutes late for your session you will be entitled to a complimentary session, along with any remaining time you still have left for that session.
If you are late for your session, your session will be reduced in accordance with that time. If you are more than 20 minutes late, your session may be cancelled and you lose your session. In this circumstance no refund will be given.
Expiry and Extensions
All personal training packs have a 90-day expiry, after which time any unused sessions will be lost and no refund made. This is to ensure we can deliver all your sessions to meet your goals.
You may extend training packs for between one month and 12 full calendar months for the following reasons only- pregnancy - Serious illness - serious injury - redundancy. To extend your personal training packs, proof may be asked for.
STANDING ORDER MEMBERSHIP PACKAGES
This program is designed so you reach your goals so full commitment is required and prices are lower to reflect this.
You are charged the same each month regardless of how many sessions you attend. However, the prices are designed so you are not penalised, and it works out cheaper for those who commit. For
For Example, with the anytime premium package could work up to equivalent of 2.7 free sessions per month (29.7 free sessions within a year) compared to standard packages. This should cover if you cannot manage sessions due to holidays or need to miss a session or 2 because of other commitments and still works out cheaper. The standard anytime works out at the equivalent of 1 session free per month or 11 per year (this includes trainer’s standard 4 weeks off per year, if the trainer takes any additional time off sessions missed will either be refunded, or extra sessions given at client’s request).
Dates, days and times will be agreed during consultation. Sessions can be changed prior to minimum of 24-hour notice for that week, subject to availability.
There is a 3-month minimum commitment to all standing order packaged. 3 full months must be completed before any cancellation of sessions. If you have completed 3 full months of standing order sessions personal and chose to finish your sessions going forward you must give notification in writing, either by email or letter.
Sessions can be suspended for medical reasons, injury, pregnancy and redundancy.
Top-up sessions for standing order packages are available at any time and can only be purchased by those committed to standing order training option.
Refunds
If for any medical reason you are no longer able to complete any personal training sessions, you will be given a refund on any outstanding sessions, we may ask for proof from your GP. If you decide to stop training for any reason apart from medical reasons or redundancies, you will not be entitled to a refund.
Informed Consent & Assumption of Risk (Must be signed prior to beginning personal training sessions)
I, the undersigned, being aware of my own health and physical condition. There are always certain risks associated with any physical activity. I understand these risks and declare myself physically sound and capable to participate in the program from UK Personal Training. I will declare any current and/or previous medical conditions or injuries to my trainer before commencing the first session. The Personal Training Program is a program designed to guide me, safely and effectively, through an appropriate individualized fitness/exercise regime based on my initial fitness assessment and goal assessment. I realize that I have the option to discontinue any activity upon my own discretion. I also realize that all information obtained about myself through this program will be kept in strict confidence within the Personal Training Program.
